Description

Purpose of this position:

The purpose of this position is to assist in the administering of medications to residents as ordered by the attending physician, under the direction of the attending physician, the nurse supervisor or the charge nurse, and the Director of Nursing Services. The administering of medications will be in accordance with established Certified Medication Aide standards, the policies and procedures of this facility and the requirements of the state of Texas.

Qualifications: Must possess a high school education or higher. Must be hold a current Certified Medication Aide Certification having successfully completed a state approved training program and the necessary exam in accordance with the laws of the state of Texas.

Delegation of authority:

As a Certified Medication Aide you are delegated the administrative authority, responsibility and accountability necessary for carrying out your assigned duties.

Quality Assurance/Performance Improvement Responsibilities:

  • Recognize your role as part of the QAPI efforts of your organization.
  • Attend trainings to build understanding and capacity to undertake QAPI work.
  • Carry out QAPI roles and responsibilities as assigned.
  • Follow established policies and procedures in support of QAPI efforts.
  • Look for and share with leadership ideas for improvement in the organization.
  • Communicate to leadership upon witnessing a positive outcome of a QAPI project or detecting barriers preventing project success.
  • Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude), including adjusting performance and practice in accordance with QAPI initiatives and findings.

Compliance Responsibilities:

  • Observe all facility policies and procedures to assure resident and employee safety.
  • Observe infection control procedures to assure resident and employee safety.
  • Observe facility Residents’ Rights policies and procedures.
  • Observe facility safety policies and procedures to assure resident and employee safety.
  • Agree not to disclose assigned user I.D. codes and passwords for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Administrator.
  • Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
  • Report any known or suspected unauthorized attempt to access facility’s information system.
  • Report suspected or known incidence of fraud relative to false billings, cost reports, kickbacks, etc.
  • Ensure that computer workstations are properly logged off when left unattended.
  • Immediately report all suspected or alleged abuse, neglect exploitation and/or misappropriation to the Administrator and/or designee. 
Requirements

Job duties include but are not limited to:

  1. Participate and cooperate with any facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing Services.
  2. Participate in Quality Assurance programs, the Pharmaceutical Services Committee, and any other facility committee or program that affects the medication pass and seeks to improve the performance and accuracy of the medication process.
  3. Accurately and safely prepare, administer, and document the oral and/or topical medications that are commonly used in this facility and that may be ordered for resident use by the attending physician or the Medical Director.
  4. Follow the labeling policies and practices of the facility.
  5. Ensure that all medications administered are properly labeled.
  6. Follow facility procedures in regard to charting of medications.
  7. Follow facility procedures governing drug receipt errors, including notifying the delivery source and the Director of Nursing Services or Assistant Director of Nursing Services.
  8. Assist in documenting and removing medications that are discontinued by the attending physician.
  9. Ensure that unused/discontinued medication is returned to the pharmacy according to facility policies and procedures.
  10. Accurately measure, record, and report the vital signs of residents.
  11. Observe and verify that medication is ingested or applied as directed. Document any instance where prescribed medication is not administered, including reason(s) for refusal.
  12. Promptly notify the Charge Nurse, Nurse Supervisor, or Director of Nursing Services of any errors or reactions to medications by residents.
  13. Verify the identity of the resident before administering the medication treatment.
  14. Follow the facility’s procedures allowing residents to self-administer his or her medications.
  15. Assist the Nurse Supervisor/Charge Nurse in monitoring and documenting all aspects of such self-administering.
  16. Promptly notify the Nurse Supervisor/Charge Nurse of any failure of a resident to properly take any “self-administered” medications.
  17. Follow established guidelines concerning the storage of drugs and biologicals.
  18. Be familiar with the meaning and usage of common medical abbreviations, symbols, and terms relative to the administration of medications as used by the facility, pharmacists, and physicians.
  19. Attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as to maintain your certification on a current status.
  20. Attend annual in-service training programs as scheduled (e.g., OSHA, TB, HIPAA, Abuse Prevention, Safety, Infection Control, etc.).
  21. Use personal protective equipment when performing procedures that involve exposure to blood or body fluids.
  22. Report all hazardous conditions and equipment to the nurse supervisor/charge nurse immediately.
  23. Follow established safety precautions in the performance of all duties.
  24. Wash hands before and after performing any service for the resident.
  25. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.
  26. Report any complaints or grievances made by residents to the Director of Nursing Services.
  27. Knock before entering the resident’s room.
  28. Report incidents or suspected incidents of resident abuse to the DON immediately.
  29. Honor the resident’s refusal of treatments or medications request.
  30. Report a resident’s refusal of treatments or medications to the nurse supervisor/charge nurse.
  31. Follow fire and disaster procedures during a drill and actual occurrence.

 NOTE: The job description for Certified Medication Aide may include any function delegated by the Charge Nurse, Assistant Director of Nursing Services and the Director of Nursing Services.   

 Working Conditions

  • Works in office area(s) as well as throughout the nursing service area (i.e., drug rooms, nurses’ stations, resident rooms, etc.).
  • Moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
  • Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
  • Communicates with the medical staff, nursing personnel, and other department directors.
  • Works beyond normal working hours and on weekends and holidays when necessary.
  • Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
  • Is involved in community/civic health matters/projects as appropriate.
  • Attends and participates in continuing educational programs.
  • Is subject to injury from falls, lifting, burns from equipment, odors, etc., throughout the workday, as well as to reactions from dust, disinfectants, and other air contaminants.
  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
  • May be subject to the handling of and exposure to hazardous chemicals.
  • Maintains a liaison role with the residents, their families, support departments, etc., to adequately plan for the residents’ needs.
  • Follows established procedures governing the handling and disposal of infectious waste.

 Specific Requirements

  • Must be able to read, write, speak, and understand the English language.
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • Must be a supportive team member, contribute to and be an example of team work and team concept.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must possess the ability and willingness to work harmoniously with and other personnel.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be able to relate information concerning a resident’s condition.
  • Must provide documentation of such certification upon application for the position.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.

  • Must be able to move intermittently throughout the workday.
  • Must be able to use fine motor skills with hands and fingers to carry out assigned duties.

  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.

  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
  • Must meet general physical and health requirements to perform job duties.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
