Cook | Family Health and Rehab

The Family Health & Rehabilitation Center is a unique Skilled Nursing Facility, designed to give each resident a sense of home and independence. Unlike traditional Skilled Nursing settings that have long double loaded corridors, the Family Health & Rehabilitation Center is built much more like one’s own home, where bedrooms are located short distances from the living and dining areas. The center features four houses each with 16 private rooms and 2 semi-prive rooms for a total of 72 rooms. Each house has its own dedicated staff, living area, dining room and kitchen. The facility is staffed twenty-four hours each day and provides a wide variety of care to residents needing skilled nursing intervention. When it is all said and done, everything we do is engineered to Improve Lives and Exceed Expectations!
