As the Social Services Director, you will perform duties and implement programs which meet physical, mental and psychosocial needs of residents. The Social Services Director is also involved in marketing and tracks, communicates and completes numerous forms for admissions. Ideally, this person will be a highly organized individual with great communication skills. 


You will report to the Executive Director.


Day-to-Day Responsibilities:


  • Gives tours of the facility to potential residents and their families.
  • Coordinates all admissions to the facility. Including readmissions, room movies, and patient inquires.
  • Coordinates all discharges. Including written discharge plans.
  • Responsible for Social Service progress notes, assessments, and care plans.
  • Actively participates in care plan meetings and may be responsible for involving residents and family members.


General Responsibilities:


  • Helps investigate facility concerns regarding allegations of abuse, neglect, and exploitation.
  • Provides information for the Quality Assurance Committee weekly and monthly by documenting and reporting potential and actual issues that affect quality care and services.
  • May be required to develop and conduct psychosocial resident groups.
  • May be required  to coordinate resident and family member grievances and act as the resident advocate when necessary.
  • Assures compliance with all State and Federal requirements regarding admission and Social Services; as well as state and personal compliance to all State and Federal regulations including bloodborne pathogens, infection control, use of hazardous materials and fire safety; and Resident’s Rights.
  • Other duties as required.

Education & Certifications


  • Master’s Degree in Social Work
  • Colorado Social Worker license




  • 3+ years of Social Work in Healthcare
  • Social work in skilled nursing preferred
