Summary: Guide families entering Help Me Grow Early Intervention through the eligibility determination process as well as coordinate services throughout the family’s involvement with Help Me Grow Early Intervention (HMG/EI) which address child and family needs.

Description

 Essential Duties and Responsibilities:

  1. Engages families and arranges and completes home visits, determines eligibility, and connects families to specialized HMG/EI services.
  2. Facilitates and participates in developing and monitoring each Individualized Family Service Plan (IFSP).
  3. Regularly coordinates and/or conducts developmental evaluations and family assessments during home visits to determine eligibility for services and program planning.
  4. Participates on a multi-disciplinary team to deliver seamless services to families eligible for Part C.
  5. Communicates regularly to team to discuss family and program concerns, issues and successes.
  6. Conducts required screenings to determine eligibility for Part C and other related programs.
  7. Informs parents and/or caregivers of their rights and ensures all consent and required forms are completed.
  8. Maintains documents, files and prepares reports on each family as required by the State of Ohio, Wood County and local agencies and initiates follow-up as necessary.
  9. Provides advocacy, education and identification of services as a HMG/EI representative.
  10. Participates  and facilitates community and staff development training events.
  11. Maintains credentialing status, as required by Ohio Department of Developmental Disabilities, by attending and completing all required training and testing.
  12. Interacts professionally on a regular basis with staff, vendors and the general public.
  13. Ensures that family meets all timelines including initial 45 days, timely receipt of services and transition activities.
  14. Other duties and responsibilities as directed by supervisor.

 

 

 

Requirements

Education:

Required Associate degree in Early Childhood Education, Social Work, nursing or related field; prefer a Bachelors degree.  Prefer Service Coordinator Credential or must have Service Coordinator Credential within six months of hire and must maintain without lapse and renew every two years by completing at least twenty contact hours of training related to HMG.

 Experience:

Three to five years experience working with families and children/conducting home visits, community resources and local education agencies and up to one year experience with word processing, spreadsheet, internet, and database software.

 Additional Requirements:

  1. Initial and periodic Motor Vehicle Records checks, maintain a valid driver’s license, and maintain a safe driving record.
  2. Must have insured, dependable transportation available for daily use.
  3. Initial and periodic physical exams required.
  4. Must submit to initial and/or periodic state and/or federal criminal checks.
  5. Must submit to initial and periodic drug and alcohol testing.
  6. Ability to be reached by cellular phone during business hours and after hours.
